TESA
TESA, or Testicular Sperm Aspiration, is a pioneering sperm retrieval technique primarily for men with Azoospermia, a condition where sperm is absent in their ejaculate. TESA is employed in obstructive Azoospermia, where blockages prevent sperm from reaching the semen. This technique, coupled with intracytoplasmic sperm injection (ICSI), has expanded the possibilities for men facing azoospermia and offers an avenue to parenthood when sperm cannot be retrieved from the epididymis.
PESA
PESA, Percutaneous epididymal sperm aspiration is a procedure done under local anesthesia where the contents are aspirated from epididymis and examined for presence of sperms under microscope. It is a simple, quick procedure which can avoid open surgery.

Microdissection TESE OR Micro TESE:
Microsurgical testicular sperm extraction is a procedure where less tissue is removed with less testicular damage. Seminiferous tubules are separated and examined under operating microscope for sperms after biopsy.
There is improved chances of finding sperms in cases of focal spermatogenesis.
Process
Spinal anesthesia administration
Spinal anesthesia is administered to ensure patient comfort during Testicular Sperm Extraction (TESE) procedures.
Sperm extraction
Testes is exposed by midline incision and the seminiferous tubules are separated and examined meticulously under operating microscope.

Healthy seminiferous tubules are biopsied for sperm extraction
Injection of the sperm
ICSI, a fertilization procedure, is then employed to inject the collected sperm directly into the egg and facilitate fertilization.
In Non obstructive Azoospermia cases where needle aspiration biopsies fail to identify sperms, micro- TESE is the ultimate option. But undergoing repeated failed attempts in non microsurgical procedures, then followed by decision for Micro TESE is not cost effective, frustrating and time consuming.
